New Patient Registration

Welcome to the Burwell Surgery!

We care for patients in Burwell and the villages of Exning, Reach, Swaffham Prior, Swaffham Bulbeck and Fordham.

If you would like to register with us, please complete and return your registration form GMS1 along with the new patient questionnaire. It is very helpful to supply your NHS number, which you can obtain from your old surgery.

We will require two forms of valid identification for each new adult applicant, so please bring original documents for this purpose.

Examples of suitable identification include:

Photo identity – current passport, driving licence. Address identity (within the last three months) – utility bill, council tax bill, rental agreement or solicitor’s letter.

Each application on behalf of a child (16 years and below) will need to be accompanied by either a birth certificate or medical card.

To register new born babies, please bring your delivery discharge form and ask for a GMS1 to complete.

As a new patient you will be offered a health check with one of our practice nurses. This check includes a blood pressure check, lifestyle assessment and family history. All of these factors can influence your health and we feel it is a good opportunity to address any issues that you may want to discuss.

If you are taking any regular medications, we ask you to book an appointment with a GP before you need to request a repeat prescription.

We would also like to bring to your attention our Online Services section of the website. By registering for online services, you can make appointments and order repeat prescriptions online. Please ask our Reception team for details of how to sign up if you are interested.

Accessible Information: Making Information Clearer for Patients

Here at Burwell Surgery, we are dedicated to the needs of all our patients, as such we would like to ensure that all information we send to you is clear and that you are able to understand it.  If you feel you would like additional support please let us know.

Changes to Personal Details

Please notify the practice as soon as you can of any change of address or telephone number, so that your records can be kept up to date to ensure that reminders for blood tests, health checks, smears etc. go to your correct address. It is also important that we have your correct address and telephone number if the doctor needs to make a home visit.

If you move out of the Practice area the doctors are no longer responsible for your health needs. We therefore advise you to register with a Surgery in the area you live as soon as possible. Please ask at reception for further details.
